There are 4 types of form (all available from the resources on the left hand side of the screen) and the type of form to be used is dependent on which diocese you are in and whether you changed your appointment and house in the period April 2008 to March 2009.
If you are in Bath & Wells, Blackburn, Bradford, Chichester, Guildford, Lichfield, Liverpool, Manchester, Newcastle, Portsmouth, Rochester, St Albans, Salisbury, Sheffield, Southwell & Nottingham, Winchester, Worcester or York please use:
Type A if you did not change appointment and house; and
Type B if you changed appointment and house in the period.
If you are in Birmingham, Bristol, Canterbury, Carlisle, Chelmsford, Chester, Coventry, Derby, Durham, Ely, Exeter, Gloucester, Leicester, Lincoln, London, Norwich, Oxford, Peterborough, Ripon & Leeds, St Edmundsbury & Ipswich, Truro or Wakefield please use:
Type C if you did not change appointment and house; and
Type D if you changed appointment and house in the period.
